Waiver & Liability Release

By registering for a Proud Human Hoops League (PHHL) 3x3 event, I acknowledge and agree to the following on behalf of the registered athlete:

I understand that participation in basketball and related athletic activities involves inherent risks, including but not limited to physical injury, illness, or other unforeseen harm. I acknowledge that these risks may occur despite reasonable precautions.

I voluntarily assume all risks associated with the athlete’s participation in PHHL 3x3 events, including warm-ups, games, workshops, and any related activities.

I hereby release, waive, and discharge Proud Human Hoops, the Proud Human Hoops League (PHHL), its directors, coaches, staff, volunteers, officials, contractors, facility operators, and partners from any and all claims, demands, actions, or causes of action arising out of or related to participation in this event, except in cases of gross negligence.

I confirm that the athlete is physically able to participate and has no medical conditions that would prevent safe participation, or that any relevant conditions have been disclosed.

I grant permission for the athlete to receive emergency medical treatment if required and understand that I am responsible for any associated costs.

I also grant Proud Human Hoops permission to use photographs and video recordings of the athlete taken during the event for promotional, educational, or marketing purposes, unless otherwise requested in writing prior to the event.
